Paws In Focus “PIF”

I, (indicating all those in the household) HEREBY WAIVE AND RELEASE (Pet name) from
liability pertaining to the matters set forth below. I understand that by signing this Waiver and
Release, I expressively and willingly agree to assume complete responsibility for any risk of
injury, personal property damage, pet loss/theft, or death that may arise while at Paws In Focus
“PIF”, (including Lindsay Hatt, Ryan Quinn Wallcraft, any employees, directors or anyone
employed at Paws In Focus “PIF”.) is watching, training or boarding my pet(s).

On behalf of myself and all owners in the household, I waive all claims for damages, injuries,
diseases, pet loss/theft and death sustained to my pet(s) or my property, that I may have
against Paws In Focus, (including Lindsay Hatt, Ryan Quinn Wallcraft, any employees, directors
or anyone employed at Paws In Focus) relating to programs and boarding offered by Paws In
Focus.

I understand that my pet could be lost or escape, be injured during play, become ill or die while
at Paws In Focus.

Paws In Focus home, property, or personal belongs could be damaged during this time.
By this waiver, I assume any risk, and take full financial responsibility for damages to personal
property, injuries, diseases, loss/theft, or death pertaining to my pet (s) associated with Paws In
Focus, including, but not limited to, cuts or puncture wounds, scratches, chewing on personal
belongs, walls, or flooring, accidental disease from outside or the trainer themselves, and loss
of the pet by escaping or stolen.

I acknowledge that is is my responsibility to provide Paws In Focus (including Lindsay Hatt,
Ryan Quinn Wallcraft, any employees, directors or anyone employed at Paws In Focus) with
accurate and up to date medical records for my pet. That these medical records will include,
but are not limited to: up to date vaccination records for Rabies, DHPP, Leptospirosis, and
Bordetella, medication list, microchip number, and medical history.

I am also agreeing to having my pet on veterinary recommended parasite preventative for flea
and ticks, and that PIF is in no way responsible for any parasite received or transmitted to me,
my pet or my household.

If my pet or property are harmed during the duration of the stay away from home (including
transportation to and from), I will not hold Paws In Focus responsible even if the injuries,
damages, illness, loss/theft or death were caused during the care of Paws in Focus, or any
other party under or affiliated with Paws In Focus.

I have informed Paws In Focus of any physical limitations, ailments, or physical/mental
disability my pet may have that would cause him/her to be uncomfortable, aggressive, or
contagious towards other animals or people.

I understand that any advice, counsel or training received is subjective, and release Paws In
Focus (including Lindsay Hatt, Ryan Quinn Wallcraft, any employees, directors or anyone
employed at Paws In Focus) of any responsibility of outcomes from said advice, counsel or
training. I acknowledge it is my responsibility to consult my own medical practitioner if I have
any health, safety, nutrition or supplement concerns with my dog, my family or myself.
I acknowledge that PIF uses a variety of training tools including but not limited to; clickers, slip
leads, prong collars, & remote collars. If PIF believes that you are using these training tools
incorrectly, we reserve the right to take back tools that were provided to you by PIF.
I understand and agree that there are no refunds for any reason at anytime on any service
provided or items sold. That results are dictated by my own implementation of information and
training.

I HAVE READ AND FULLY AGREE TO THE TERMS OF THIS WAIVER AND RELEASE.
I UNDERSTAND AND CONFIRM BY SIGNING THIS WAIVER AND RELEASE I HAVE GIVEN UP
FUTURE LEGAL RIGHTS. I HAVE SIGNED THIS WAIVER FREELY, VOLUNTARILY, UNDER NO

THREAT OF DURESS WITHOUT INDUCEMENT, PROMISE OR GUARANTEE BEING
COMMUNICATED TO ME.
MY SIGNATURE IS PROOF OF MY INTENTION TO EXECUTE A COMPLETE AND
UNCONDITIONAL WAIVER AND RELEASE OF ALL LIABILITY TO THE FULL AND EXTEND OF
THE LAW.
I AM 18 YEARS OF AGE OR OLDER AND COMPETENT TO EXTER GRANT THIS WAIVER.

I declare that the foregoing to be true and correct.
